I hate low-poly shit with awkward animations and Pathologic is full of this shit
The game makes you feel more dread than fear but at this one point early on you enter a U-shaped infected house, talk to a woman who tells you her sisters are shambling around, so you assume they're deeper into the house. The game actually spawns the two infected sisters behind where you came in and if you immediately turn around you see one just shuffling towards you like this.

Think the traditional zombie infection, but worse.
Instead of dead reanimated corpses, people are exposed to the flu, and undergo transformations that happen at a variety of speeds, similar to rabies in animals, ranging from a few seconds to a few hours. You don't have to be bitten to become infected, all you need to do is be near someone that has the flu. Rather than seeing humans as food sources, they're seen as monsters to people who're turning into, or are, infected. Infected are known to bite, but they never eat the corpses, the ones you usually see in pretty much every map in the game. The survivors are genetically immune to the green flu, and can take much more punishment from the infected before kicking the bucket.
Special infected go through more specific mutations, usually caused by a habit in their human form (smoker, hunter, boomer), or taking unnatural, body-shifting supplements (tank, charger). In the case of the witch, they're thought of to have had some form of mental illness or disorder before turning infected, like depression or schizophrenia, possibly anorexia, looking at how thin they are. There is no concrete answer on exactly why the witch is crying, but it's either thought of as the infectee realizing what they are, and weeping day in and day out until disturbed and killed, or a way to lure survivors in for them to kill them.
tl;dr, the green flu is essentially a more realistic depiction of a zombie infection, and it's even worse than the hollywood films.

I don't get this. On every climbing part or anywhere you need to use/can use the rope bow there hasn't been an enemy hidden somewhere to kick you off. There's some treasures and secret paths in underwater dungeon areas. Shipwrecks also usually require it.

There's one big quest where you need to go to a sunken city full of those things. That's probably the worst of it.
