The horn "grows" as an indicator of how many people you kill in MGS5. if you kill alot of people he also gets permanently covered in blood.Its actually really annoying since the game basically forces you to look like a walking used tampon unless you go completely non lethal or fucking keep visiting and leaving the animal sanctuary to get your "good-guy" points back up.

That's why they use a ton of ways to break those patrols and use diversions.

You have like a dozen equipment slots, and there's more than 12 non-lethal equipments for sure. And lethal weaponry can also be used to destroy equipment. So I don't see why you're complaining.
