I recently started another playthrough and I had issues where Geralt would just stop moving in combat and interface wouldn't work aside from attacking with mouse, I couldn't open any panel or anything, either with mouse or hotkeys; had another problem in the jail at the start of Chapter 2 where I couldn't talk to anyone and had to restart the game to get it to work again.

Was pretty odd.
