That's how most of them are. There are actually ways to make them properly, but a lot of the modders are extremely lazy. It's pretty irritating, because almost all of the skirt mods don't have underwear as a result, unless you have a set underwear texture painted on like that, which looks like shit. Most of the underwear mods that are actually equipment take up the entire body slot, meaning wearing it means running around in panties and a bra.

There actually are a couple decent underwear mods that work like intended, but they're few and far between.
