Check The Void. No game has examined the issue quite as well and in complex detail, and rather uniquely through actual gameplay mechanics.

TES famously has some gnostic-inspired elements of it's lore, but it's not exactly focusing on the existential dread, but rather on the novelty of Gnostic divine hierarchies and shit.

Other than that, I'm kinda out of ideas. I don't think it's really possible to convey the actual idea of existential dread that Buddhism tackles in any meaningful way through modern gameplay mechanics in a way that would be elegant and not hamfisted and awkward, or would require near infinite amount of resources to pull off. You would first need to establish the totality of being to then confront the player with the dread that comes from it, and that... well I can't think of any way to really achieve that.

That said, Japan just has less of self-censorship breaks in general. Look at their pornography, their exploitation cinema, but also at their fucking comedy or drama. They just don't really feel like there is any reason to shy away from any type of niche-oriented fascination (no matter how sick it may turn out to be) in their media as long as there is an audience.

They are less cautious about exploring/depicting the deprived shit that all societies dream about, but western one is more used to pretend they don't.

I want to make a game about some nigh-omnipotent-through-meta-plot fucker like the Doomguy who goes around all day, busting into pocket dimensions and shit to give people subjected to eternal-suffering type punishments guns and the power of Rip and Tear demons without regret.

The demon slaying would be easy for him. The challenge is convincing the sufferer to return from a state of resignation to a state of wanting to SLAY.

And all of the missions would be direct references to things like Demonophobia and various other shitty RPGmaker/GameMaker games and stuff. Literally anything that ends with, "uh oh eTernAL sUFFeRiNG!11!!1!!" and the lore of this game will go through every single autistic detail over why this not-Doomguy will literally win every single time and go into a huge, never-ending power-wank that no random indie dev is going to be able to say, "no but you see actually I can make something evil that is more powerful." No, just unleash the chains of full-on Demonbane-level "you can't actually surmount this ultimate good" and call it a day.

There is an abode of the dead - divided into two place, one for righteous and one for wicked, which further reflects an absolutely key passage in Psalms and Isaiah specifically stating that those faithful will be rewarded in the afterlife, and will dwell by Gods right hand, while those wicked will be punished by his left...
