Spess muhreens are mutants, genetically enhanced and modified to the extreme. I cant remember everything thats done to them, but they go through so much that very few actually end up becoming Astartes (which, in turn, is damn near close to being a demi god). Everything from muscles to reinforced bones, extra organs, implants, etc happens to create a Space Marine
