Crouch if you want better accuracy. But yeah, the STALKER games are known for not being the kind to have weapons always shoot down the crosshair or iron sights. If you want to kill things with a crappy pistol, you have to get a bit closer and crouch so your shots are less likely to be off mark. By Agroprom though, you should have already thrown your pistol away and be using a shotgun for mutants and an SMG or an assault rifle for human enemies.

If you're literally playing vanilla you need to set the difficulty to Master otherwise bullets randomly vanish out of existence upon impact for both you and the enemies.
