Ultra Violence, normal is too easy, especially on the PC, with UV giving you just enough chances to feel like a goddamn rape train, but with enemies having just as many chances as well

Although even nightmare can be somewhat cheesed if you chain the skills that give you armor shards after executions, faster executions, and high armor providing infinite ammo, that combo is broken on ANY difficulty, especially once you get siege mode for the gauss cannon and mobile turret for the chaingun

Ensure shotgun grenades and the scope for the AR are the very first upgrades you tend to
