You learn the maps, you learn how each weapon works, and you increase your ability to react quickly and determine what actions the enemy will take. I played with someone like this back in Halo Reach. He'd form full parties to go pubstomping, then he'd start off by going super tryhard to make a few quit so he could set up easy spawn traps for the remaining few. I had to unfriend him after a week. I had this normie friend in high school I'd play halo 2 with. He had multiple xboxes set up in his house. He was pretty good but for some reason decided to install this cheat program our other friend showed him. You could edit all the stat values for the guns and make any vehicle fly and crap. One day we sat down and just fucked with people by making them fly across the map with a single pistol shot. Hacks can be pretty fun. There have been some really neat features for messing around with other players without just ruining the game for them, especially in MMOs and non-competitive or non-arena games. Even CS1.6 had some weird stuff like third person modes and ways to manipulate other players via observer.

A lot of people using hacks tend to use from the same source, so I'm pretty sure it checks if the processes is being fucked with and watches them for a while.

I do think that there is some safeguards to prevent false bans, one of them prevents them from playing VAC games at all. My friend uses a third party task manager (he says it runs better than the Windows one) but that prevents him playing Dota 2 with us until he disables it since lets him examine the actual processes of Dota 2, so I assume similar shit is in place.
