I used to do this with a mod Fallout 3. I would use child races and then change my spawn point to Little Lamplight and role play like I was from there at the beginning of the story. I doubt Bethesda would allow for something like this in a modern Fallout game.

How are they keeping their population if adults leave it?

How about a ~70years pre fallout 1 where you have to survive nuclear winter and storms.

Say you are basically the forerunner of the ghouls and basically you get to see the very beginning of many factionsand species, maybe open a sealed vault or two.

Honestly though fallout 4 ruined fallout, imo not sure how it can come back from that
