Inafune did design the spritework in MM1, and made final creative changes to the concept art for which he published. But it wasn't his base design, but I'd sooner focus on that he didn't lead the first game's development.

You can argue points from that, but really it was his NES era creative leading that got him the title. Inafune later took some production lead across the franchise, including licensing with the Ruby Spears cartoon, but he was the one that went out to give interviews, attend events, and announced the submission contest winners. People saw his name on everything, saw him in every creative venue and well that's kind of how a moniker goes.
