How is it done right, though? The games I've played that had it used it as either an effect of some insanity mechanic, or as something that a boss does. It definitely messes with you, but it also completely breaks the immersion.

It honestly didn't even make sense in Undertale. At least when Conker did it, it had some kind of payoff because it ended up being part of the plot. In Undertale, the final boss can cause the game to crash and save and load but it's never really acknowledged in any way. It's just something that happens for no reason.
