Actually I don't know why they chose to start out with a Goomba rather than putting a block in the player's way. Maybe they wanted to cement that "Enemies are lethal" right from the get go?

The original NES controller only had 4 buttons and a D pad. It was feasible then to learn a games functions just by pressing buttons and seeing what they do, but it isn't now since controllers now have many more buttons.

Tutorials are a necessity in this modern era, since games demand a lot more functions, regardless of whether they were easy or hard, but weren't then, since they had simple functionality, even if the games themselves were difficult.

Brilliant level design encourages new players to stomp on it. If you touch it, you die. Most new players aren't going to be masters of the B button to dash, and holding down A to jump to maximum height. The bricks above the Goomba make sure an inexperienced jumper isn't going to get high enough to go over, and bumping your head makes Mario fall down faster which increases likelihood of death. After dying a few times, it's only natural to figure out you need to fight the Goomba for easier access to the rest of the level. B appears to do nothing but makes death come faster, and A jumps. Well, B doesn't work so there must be something to do with A. Wa laa.

Miyamoto once explained how he designed 1-1. He said that originally goombas didnt exist and the first enemy was a green koopa troopa. But later he playtested it and many people jumped twice on them and killed themselves with the shell bouncing on the pipe, so he created the goomba as first enemy so the player can understand how to kill an enemy in the game.
