Don't quote me on this, but I'm pretty sure it started in 1993, with Mario 64 always planned as a launch title.
The whole game got scrapped about a year before the 64 launch because the team decided it wasn't fun. The Bowser stages are all remnants of the former level design. Everything else was basically cranked out in the months leading to launch since they refused to back down on the established launch date and simultaneous release.

Its actually pretty cool how they look like captain toad diorama levels. I think the development team talked about in an interview how one of the early version of mario 64 played out with the camera being rotated around the stage similar to captain toad at one point.

Because Mario 64's levels were designed very carefully around the camera. Hacks like Last Impact are technically impressive but suck to play because they don't have the quality of the original games level design.
