What is, in your opinion, the single most difficult puzzle to figure out without using a guide?
Nominating this terrible, nightmarish thing which you have to do right at the start of Chamber of Birth.

Monkey Island 2 - Monkey wrench
I only solved it because for some fucked up reason I remember reading a review of this game a decade before I played, and I distinctly remembered the reviewer mention the monkey wrench puzzle as being bizarre.
