Canonically via interview the symbol is based off of Terra's.

How they obtain them is never explicitly said in game or interview, however the manga and novel (game too when you remember the dusk at the start opened its suit up to talk) imply that they are ALL in costumes.

So basically depending on power level they start as formless blobs barely smarter than heartless (creeper) and get more humanoid (Dusk) and then get a assigned and org member and are given weapons.

No, Emblems ARE the heart, enveloped by darkness.

Purebloods and Emblems both seek out and steal hearts, but Emblems are actually made from a heart. The heart that is released is the heart that was corrupted into the Emblem, not one that it had taken from someone else.

It's also worth noting that Xehanort is the one that created the Emblem, so Keyblade wielders who were lost to darkness during the War would not have emblems on them.
Though we don't really know the full story on the Darklings either way. Somehow the Nightmare was able to absorb them, which is a weird power for ostensibly completely different species.

The Darklings may be the "first" Heartless, and as a result behave differently, or even be the cause of Purebloods (similar to Vanitas spawning Unversed).
