Gameplay is exactly the same as 3D's with updated graphics.
The twins don't die and Duke doesn't look like a psycopath. More babes in general
Some slight tongue-in-cheek and maybe parodying more tropes of buff 80's action stars.
Driving sections suck gargantuan dick.
More basic stuff I'm not mentioning.

The point is that it's not hard to do a Duke Nukem game. They fucked it up by getting too ambitious and focusing on a million different shit that's not shooting aliens, getting babes and oneliners.

The only way to fix DNF would be to start over. Its problems all come from the fact that one piece of it was made in 1999 and another was made a in 2007, etc.

Gearbox took a jumbled bunch of old assets, loosely stitched them together and called it a day. The game is all over the place and you can see all the different trends it was chasing due to having a multi-generational development cycle. It needed a clean slate but that would have required Gearbox to spend money on something that wasn't one of their own IPs.

Remove health regen.
Remove weapon limits.
Restore them color palette.
Design maps as legitimate locations rather than hallways.

99% of the levels and features you see in it, including the regenerating health and weapon limit, are 3DR's (or George Broussard's to be exact) fault.

Shortly after they announced that the game was cancelled and 3DR was closing doors a video from one of the designer's portfolio showcasing about eight minutes of gameplay got leaked and ALL that shit was already in place.

Why do you think that 3DR never released it? They knew it was shit and they kept starting over. Gearbox took what they had and shoved it out the door without caring about quality.

The DNF we got was never intended to be released. The game probably should have just been canceled, but Gearbox saw a great opportunity to make money and then blame 3DR for the game not being up to quality, even though it was Gearbox who chose to release it in the state it was. I say the game's poor quality is entirely on Gearbox because 3DR had no say in whether or not the game would be released in the state it was, and Gearbox gave it their stamp of approval before shoving it out the door. Maybe DNF was an impossible dream that could never truly be realized, but 3DR didn't ask people $60 for it in the state that Gearbox released it.
