I don't know what constitutes "neutral", but my first run was a pacifist run. Then after you beat Flowey or whatever, it unlocks the lesbian lizard's portion of the game to get the true pacifist ending. Genocide is the third. I guess you could say the extra shit after you beat Flowey is part of the first run, but I thought there was a credit sequence after beating Flowey, which is why I counted it as a separate ending. The reason Sans never intervenes is that he made a promise to Toriel and he has the right mix of depression, apathy and insanity to want to honor that promise more than avenge his friends.

The reason he finally steps him at the end of genocide is because he starts understanding that the player will NOT reset the game after killing everyone if he doesn't prevent them from finishing it. A reset is every bit as bad as wiping everything out and not resetting, because for all the characters in that playthrough, that's exactly what it is. They are wiped out. The only way to achieve a true happy ending is to play through on pacifist and not reset.
