Recent precious to me gets are a factory sealed SWAT 4 Gold edition (I got for a dollar.) and a physical copy of 100% Orange juice, which I love very much.
I got my Pokémon Puzzle League cartridge signed by Veronica Taylor. She was very nice.

Also buying the game cost me $150 (emulated Earthbound as a kid, and it left a big impact on my childhood- always wanted a real copy, but didn't have any income until a couple years back when the price had ballooned to an average of $180) and if it were fake I'd feel compelled to spend the $250 or whatever on another a real copy.
