Basically Old Man Logan was an alternate world where the world ended due to all the villains cooperating and wiping every hero out at once, including using Wolverine against the X-Men by having Mysterio make him think every other person in Xavier's school was a villain, who he ripped to shreds.

A bunch of the world gets fucked up, Bruce Banner goes crazy and becomes a cannibal who somehow inbreeds with himself and makes an entire tribe of hillbilly man-eating Hulks, Emma Frost runs a mini-utopia with the help of Black Bolt, and the remaining United States is owned by Red Skull, who cosplays as Captain America in private in pride of his victory. Using Captain America's torn, bloody uniform. And Logan retires the name "Wolverine" out of shame for what he did.

He then goes on an adventure with Hawkeye, who is now blind and uses echolocation to do awesome shit on some mission where Logan kills Red Skull, read the fucking comic and you'll enjoy it, I promise.

That's where the original "Old Man Logan" run is set.

After that came a new run after an event where basically all the worlds combined and is ruled by Doctor Doom. I won't explain all of that, but basically Old Man Logan travels across a fuckton of worlds, including one that's run by Tony Stark, until he reaches the proper universe, where he tries to figure out what the fuck is going on and looking back on his past. Then the event ended but Logan was still in the "Prime" world, so he's still going around as Logan.
