So I've been trying to understand this as well.
To put it basically western gamers aren't used to games with slightly prolific sexual content no matter how tame because it rustles their jimmies.
However people who play eastern games like nier, or most jrpgs are used to seeing this stuff so they're essentially desensitized and don't notice it as much.

For example when everyone was making a racket over how 2B looked I honestly didn't see the reason for any controversy.
I mean sure she's attractive, but I'm not so simple-minded and sheltered to get stuck on that and let it break immersion.
Video games have this stigma about being for children and stuff like ESRB ratings don't help that because they actively put restrictions on the content that can be introduced to a game.

tl;dr people who play more eastern games become desensitized to sexual content overtime and barely notice it, people who exclusively play western games are extremely sheltered from sexual content in games so seeing any slight hint of it triggers them
