You can't fix them. /v/'s complain about open world games are that they lack content and are repititive. But irl it's the same shit. Every other block it's the same shop, selling the same shit. The same no name people you'll never know who might as well have had the same face.

By scaling down the worlds and removing fast travel (except for specific points where a significant payment is required). Add plenty of secrets to discover that aren't marked on maps as a question mark. Give people a reason to explore. Make each area hand-crafted (no algorithms for automatically generating terrain and forests). Add ways to make more permanent changes, such as revitalizing a dying village or destroying a bunch of rocks to create a new river.

Make better procedural generation scripts. One that will make worlds and towns that are interesting of themselves.
Script in manually crafted towns/districts/dungeons/areas etc. Have fun game mechanics like flying, diving, character transformation, good combat, etc.
