The way that hell's society works is interesting.

Reading most of the stuff now, it says Doomguy came in during the age of the first lord of hell. Now it is the age of the forth lord of hell according to the Baron of hell datapad. So, either the Spider Mastermind or the Icon of Sin was the first lord of hell and Doomguy killed the rest or the next lord in line killed the previous.
