It's not that hard to understand, though the picture could make it clearer. The helicopter model is highlighted to prove all scenes happen in the same helicopter.

Before the crash, Big Boss has a clean face.
After Paz explodes not too far from the helicopter, he suffers burns on the eyepatch side of his face and forehead.

There is a scene where the 794 helicopter flies in the middle of the ocean at dawn.

This scene was carried over to the retail version of the 2nd game without editing out the contradictory face wounds, instead choosing to make the whole scene pre-rendered.

The fact that this scene was made in the first place implies the crash, and consequently coma + body double never actually happened, which makes Snake and Kaz's survival likelier all things considered.
