It's possible Zero did it. MGS4 states that Big Boss and Zero were enemies and had a war, ending with Boss staging a coup against him in Outer Heaven and Zanzibarland (so Zero sent Solid Snake in to kill Boss in Metal Gear 2). But instead the nonsensical poorly titled "truth" tapes portray Zero as the series hero as Big Boss's savior.

It's unclear exactly what happened but something very fishy went down at Camp Omega. Zero might've captured him and shoved that thing into his head as a way of manipulating him. As someone in prior thread once said, it's the perfect cover. Wouldn't want to touch that shrapnel in your head now, wouldn't want to fuck up your brain eh, goy? I really believe TPP is a coma nightmare / VR simulation. Maybe Zero is inducing the nightmare / VR to try and fuck with Boss's psyche and manipulate him into taking out Skullface for him.

In fact, it would only aid your theory to admit it's real, but you refuse to do so.

Listen, modeling and animating a deom like the one in Survive is not something you can do in two hours, you would have to get an artist to create the models, an actor to act you the melee combat for the mocap or an animator to do the same, plus play tester... it's a huge work load.
