What are some examples of games that are hardcore and unforgiving?

Definitely not any of the Fallout entries.

Elminage: Gothic, early Wizardry titles (like Wiz 4), some Goldbox games (Pools of Darkness), many roguelikes (ADOM). In general, early RPGs were pretty unforgiving by any standards.

Of course, there are other genres which may be very challenging, especially actiony titles (say, Super Meat Boy).
