You know that difficulty needs penalty right? The game isn't really difficult when you can infinitely respawn without losing much progression and you can just "spam" trying the maps without any thinking till you memoraze the inputs.

Zangyou mode is literally an entirely new set of levels unlocked after beating the first set, anon. They even resdesign all the bosses and make an entirely new 3 bosses, one of which has 3 phases with their own HP bars, not to mention the fact that there's an entirely new story to go with it all. Omake's extra levels are also completely unique.
The only reason anyone thinks Cave Story is difficult is because of True Last Cave/Hell/Ballos, which are a tiny portion of the game. Zangyou is at least %50 of the game total.
