The character she was in love with was "The Boss," a renowned female soldier who is the mother of Revolver Ocelot. She is Snake's mentor, aka she taught him how to fight and operate. Though Snake defeated her in the previous game, you find out she LET him defeat her, and in fact helped him throughout the game, so it is heavily implied that the iconic Big Boss is, in fact, inferior to a woman.

Why are people surprised or offended by this? The Soviet Union was all over the place doing all kinds of things for decades, they were so influential and were viewed as the antithesis to the western world to such an extent we still label countries according to their former allegiances with the west or the Soviet Union. You can take any of the hundreds of political or military events influenced by the Soviet Union and use it as inspiration for all kinds of characters and settings.
