Yes, a handful of times but it happened so rarely that I just always used the Thieves Guild to help me without using up my 3 uses of it.

It happened to me like four times total over two playthroughs.

happened to me a couple times. I just called in a snipe from another player. It never happened three times in a single dungeon so I never ran out

I think the first time it happened to me was in the 5th or 6th and was a big curveball, didnt see it coming

It happens when you are shit at video games pretty much.
An enemy needs to knock one of your party members down and they need to be in critical health, so if you pay attention it will pretty much never ever happen to you.
I went through two playthroughs without ever seeing it once and then spent a couple hours trying to force it for the trophy, it's a pretty worthless mechanic.
