Titans are born from plant world souls. These are very rare. Sargeras used to go out and search for these. Then he found one that had been infested with Old Gods, which are the physical manifestations sent into the universe by the Void Lords. They would create some sort of void titan if the planet was allowed to mature and become a titan. Sargeras decided the only way to stop this was to kill the planet (the picture where he cleaves a planet in half is this).

Titanforging should do what Asmongold said, it can only go to a certain ilvl depending on difficulty, like Normal can only titanforge as high as base mythic.
