Shame there was quite literally zero extra east coast lore added in Fallout 4. The Commonwealth was like a padlocked playpen. Absolutely no sense of a bigger world outside of the game map.

They deal with this in New Vegas. Vault Dwellers are obviously well educated, and in New California the Followers of the Apocalypse basically make it part of their mission to not only aid but also educate the Wastes. A few characters in New Vegas state that they owe their literacy/education to the Followers. IIRC even Caesar himself.

In the really isolated areas, like the national parks such as Zion and the Grand Canyon, the Tribes have developed their own languages. The Burned Man (Joshua Graham) was a translator before the Legion. If I recall correctly, the Sorrows in Honest Hearts are some mix of devolved english and Spanish. I may be wrong on that last bit though.

I think a game set in mexico, or at least partly would work, my assumption is that mexico in the fallout universe is basically filled with shoddy poseidon energy plants with waaaaaaay more lax regulations (and also greater output) that sent energy to the united states in exchange for kickbacks for the people in power, basically how our production system works now but with energy instead of cheap clothes. Fallout: Flagstaff

Legion lost New Vegas. Caesar and Lanius are dead. Who won isn't important.
Caesar had an heir back home he was going to properly mentor when he had conquered NCR, who has to rise and take control of Arizona and the Legion to stop it falling apart.
The remains of the Legion are split between following him and a highly influential Centurion who has set himself up as the new Legate and wants to rule alone.
Tribes from down south over in Mexico, who thought the Legion would steamroll them like it did everyone in Arizona before it went off to fight NCR, unite under a common banner of dismantling the Legion completely before it looks to conquer them and replenish its numbers

Twist endings: Caesar's heir becomes progressively more brutal in his attempts to exert control, Legate Newguy sees the value in not blindly following Lanius' butchery, the Nu-Triple Alliance either falls apart when it wins or becomes a Neo-Legion under the player character's rule, "Independent" is literally just "kill all the leaders" and hasn't got a Yes-Man to push it along, you just do it if you want to.

Entirely within a single vault
Have a Fallout game with a vault that was never able to be opened up and unlike 13 and 101 which did anyway because plot, this one stays shut.
The vaults are supposed to be huge but most of the games only ever show a very small part of them, with this game you could really show it in all it's glory.
With a limited number of people that can be in the vault it allows you to flesh out the npcs a lot more and also makes killing them have much bigger impact, morality could be more than just good and bad but be a question of practicality if you kill the doctor in the vault you may lose access to medical help this also makes the medical skill more worthwhile.
You could still have the wasteland exist but only interacted through radio and the occasional wanderer that comes close who you can talk to through intercom this allows you flexibility so you could set the story at any point in the series timeline.

Maybe but look at vault 13 they were threatened twice by outside forces.
regardless the point would be the internal threats like the potential breakdown of society within a vault especially if they only just found out they are never getting out.

The vaults had to be large enough to house 1000 people potentially indefinitely, they would be about the size of a small city this means they could be very large think a game map the size of maybe morrowind but far more dense in terms of content with 1000 npcs each with more detail and plot significance as the average main character in other games. Enemies would be combinations of humans, robots and creatures that had managed to get into the vault.
