Depends on what you like. Nuclear Throne is a more twitch shooter that isn't hugely dependant on the drops, at least in comparison to Isaac.

If I had to choose, I'd have to go with Isaac since I've played that longer.

NT is pretty fun, too, though. Might get back into it if Update 99 is ever released.

NT's level randomization is the best of any "top down roguelite shooter". I have 300h on NT and I'm still getting wildly novel levels and enemy placements

EtG is way too easy. EtG felt like a cakewalk to me after NT. I 100%'d it in ~70 hours. I had cleared the actual mechanical challenges of dealing with the enemies and bosses much earlier, it took me that long to get the unlocks

For contrast, I have less than half of the achievements and unlocks for NT @ ~250h

I really wanted to like it, but it just feels so fucking slow.
It seems like a lot of time has passed when you defeat the first floor boss, and by that time you've only gotten 1-2 new guns both of which could easily suck.
When it's good it's really fun. It just feels like you have to go through 85% boredom to get to the 15% that's fun. When it's an action rogue-like-like-light-whatever like this, then I want more speed, I want to be excited about what's around the corner. EtG just feels slow.

NT also loops indefinitely, so even if you had perfect game knowledge and were using a very powerful synergy (such as Fish, Trigger Fingers and Super Plasma gun), it will match your level eventually, even if takes 5+ loops

Challenge is an essential quality of a good video game to me. If it's just a casual cakewalk while I take in the sights I'm not interested

Also, NT's loops do improve the game through challenge, variation and longevity, and from offering content not present outside of loops (such as some new enemies and warps to locations like the IDPD HQ)

A loop is like 10-15 minutes, but subsequent loops can get even shorter if you have "broken" builds (like Steroids and 2x Lightning Hammer). So not very long at all. After you unlock hard mode, you can start the game on something equivalent to ~Loop 3

It's already released for a while now, it's called Nuclear Throne Together.
All the fixes that 99 was supposed to have are all already implemented into NTT plus a whole bunch of other extras (like up to 4 player online multiplayer) and even modding support with there already being quite a few mods released already.

NT hasn't been "left unfinished", if anything they had a perfect formula and started updating it too much and fucking with what people felt the "game should be like"

I for one greatly appreciate the upping of the difficulty and nerfing of babby mode synergies and characters (Chicken as it used to be, for example)

The first floor is way too long for too little rewards. It's a chore to start a game again, while in isaac you are back on rails after a maximum of 5 minutes.
