This shits me. Why does it matter if two people have the same name? It's not the username you log in with, there is no need for your in game name to be unique, ESPECIALLY if the game encourages names that fit the setting. It's perfectly reasonable for there to be two Geoffs in the world.

What are the odds of 2 guys with the same name are the same level, the same class, and the same race on the same server?
Even if by some chance that were to happen, it seems like an easy workaround for this would be to just have a character ID that would be hidden in game unless you were viewing that specific player in an "add friend" menu or something.
