generally I've read that it was meant to be an open world survival game like that other anon pointed out. The game's code reveals that the Gutter was about 5 times bigger than it actually is and has loads of unfinished stuff in it.
this is pure speculation on my part but I believe the game might have been first person as well. One of the key differences between dark souls 2 and the other soulsborne games is that many of the animations/motions were mocapped versus key framed. It's especially evident with the weapon swings, they feel kind of weightless. It was probably more acceptable in a first person view where you couldn't see the entire motion. Also the other soulsborne games uses the phyre (sp?) engine versus the new tech they developed for das2. This is also pure speculation but in interviews they talk about how they build the new tools specifically for lighting and open world design, and this doesn't really reflect the core design philosophy of the other games.
The original director of the game was "fired" but I wonder if he really just up and left after the project changed to dark souls. People have speculated that it might have been a shadow tower sequel.
Bloodborne was truly amazing. Even the general is still alive after a whole year of no new content or hopes for a sequel.

The entire game is a dream, even when you're outside the Hunter's Dream, the whole overworld is a nightmarish version of Yharnam. That's why in the worst ending, Gerhman executes you, ending your Oath, you wake up in Yharnam, essentially setting you free.

The slow build up at first matches the slow crawl the boss does before he gets to you. The third phase also coincides with a very quiet transition. The music overall makes it seem like you are fighting the most evil thing in the world.

With Dark Souls 2 you can tell how the ambition was there but they just couldn't make it. From that picture Shrine of Amana and Freja made it sort of similar, but the pirate ship was supposed to have a boss fight on the deck and it would be consumed by fire as the fight progressed rather than sink, and the Dragonriders had this nonsensical treasure room that would probably be the one in the bottom left.
Going by the trailer and the concept art I was so excited about the game, and even now I'd gladly buy something that resembled it.
