There is more than one and they were always there, you just couldn't see them before. If you get like 40 insight before you kill Rom, you will see them.

I do believe that again, if you get 40 insight before you kill Rom you can hear it, so it's possible the baby was always crying and you literally cannot hear it at all until you break the barrier
