The problem is that perfect is impossible

For the sake of argument let's say Bloodborne is flawless game aside from the Chalice Dungeons. Removing them would make the game absolutely flawless, so it could be called perfect. However Chalice Dungeons are purely optional and don't take anything out from the game so they don't make it worse. So the version with Chalice Dungeons is at least the same as the one without them, if not better because of more content. So how could a game be perfect if it could be improved?

If you go with perfect=flawless definition, you will end up with only the simplest thing, like Pong, because only those can manage to be flawless. If you say perfect means it can't be improved, everything could be better by just adding something which doesn't hurt the rest, like that video game could have one more mediocre secret level and also make you a free pizza.

I'd say RE4 but the island is a comparative let down. Dark Souls 1 is the same with Lost Izalith and Demon Ruins.

I'd say Mirror's Edge more or less perfected what it set out to do. I could give you a flaw, the story wasn't that noteworthy but who really cares?

Symphony of the Night is another where I couldn't give you a flaw.
