Do some posture exercises, basically just force yourself to stand up straight. Place your back against a doorway and bring your head up until the back of your head is touching the doorway. The 3 contact points should be your 2 shoulderblades and the back of your head.

Do this 3 times a day for 3 sets of 1 minute each.

That is not a corrective method. You need to start lifting to actually correct posture. Your abdomen is effectively deflated from disuse and your back and shoulders are much the same, you need to rebuild and maintain at least enough muscle to comfortably retain correct posture, you also need to eliminate the bad sitting habits that allowed your shit to get so fucked up in the first place.
