What don't you idiots get about free market economics? Twitch's payment plan makes it so the shithead gets $2 for each subscriber via advertisements because that's how much advertising firms can afford in order to remain competitive in their field and retain a profit, and the board of health regulations rates however much the surgeon gets according to relative need. It's fairly high sum because the surgeon is both necessary and specialised, meaning there is less of his type in the world and his place of work therefore has to offer a competitive fee to retain their services. Shithead is also "specialised" from the advertisers point of view because he's popular and thus rakes in the most revenue for them.

It's all about competition and desirability/need. There isn't some overlord arbitrarily deciding twitch streamers should make as much as surgeons whilst laughing to himself and rubbing his hands together.
