You can actually see that tower in Deus Ex Mankind Divided, albeit repositioned to a different hill than where it really is.
And yes, the babies were a temporary art installation by an artist named David Černý, an asshole who likes to piss people off by his large scale public installations. These little things were however so popular that the public made an inquiry if they could be left on the tower permanent.

Actually, I have been thinking of an open world-esque title. But to be honest, not the kind of open worlds we see today. It does not matter where contemporary open world games are set these days, they all suck because they are fundamentally poorly designed on basic levels, almost without exceptions.

I still think the genre could have a lot to offer if people would get a little more creative with it.

I would gladly take any game set in such environments though, even a fucking point-and-click if its well done.

They don't actually do that enough, as far as I'm concerned. Most open world games rely on player stumbling upon what essentially amounts to densely placed minigames thrown all around the settings. What they lack outside of more creative design of transportation, is actual emergent elements that would allow the player to actually really be creative or explorative. They lack more emergent systems and lower-level gameplay mechanics. That is the real main problem.

Caves with real verticality in their combat arenas would be nice. Deadfall adventures got pretty close. Most games with caves just use caverns are rocky hallways or set pieces with the action going on one or two flat areas.

Yeah. I've been growing increasingly fascinated with these kinds of large, open wide landscapes: steppes, deserts, grasslands. Especially in high altitudes. Not sure why, they just completely hypnotize me. Some parts of scandinavia do the same for me: regions too high or too north to have consistent woodland coverage, moss fields, open peat bogs, etc... I was talking about environments more than culture, to be honest. While you are right that the most companies around the world don't really know how to take advantage of their own cultural legacy (or are afraid of doing to avoid alienating ignorant playerbases), I'd say derivations of broadly-English medieval architecture and landscapes is pretty common to most fantasy. And - without wanting to be too much of an asshole, I'd say that more contemporary English countrysides are... not distinctive enough to be actually all that exploring-worthy.

But yeah, very few companies are actually bold and smart enough to make games based strongly enough on their own cultural legacy. Slavs tend to do that (TW, Stalker, Cryostasis, Pathologic, older ARMA titles, but also shit like Amanita games). Americans tend to use their own famous landmarks too. I know there was this one pretty fantastic Candian game that got little attention, called Sang Froid that was really utterly and thoroughly Candian.

But I do miss more culturally "distinctive" games out there.

Each par floor contains a miniboss. Every odd one is a save point. You start in the morning at the bottom and it's sunset by the time you reach the top. The room upstairs is still a letdown though. Even better if the only thing there is a quicktravel option to the bottom you can take if you know the right triggers.
