Actually, I have been thinking of an open world-esque title. But to be honest, not the kind of open worlds we see today. It does not matter where contemporary open world games are set these days, they all suck because they are fundamentally poorly designed on basic levels, almost without exceptions.
I still think the genre could have a lot to offer if people would get a little more creative with it.

They don't actually do that enough, as far as I'm concerned. Most open world games rely on player stumbling upon what essentially amounts to densely placed minigames thrown all around the settings. What they lack outside of more creative design of transportation, is actual emergent elements that would allow the player to actually really be creative or explorative. They lack more emergent systems and lower-level gameplay mechanics. That is the real main problem.

I was talking about environments more than culture, to be honest. While you are right that the most companies around the world don't really know how to take advantage of their own cultural legacy (or are afraid of doing to avoid alienating ignorant playerbases), I'd say derivations of broadly-English medieval architecture and landscapes is pretty common to most fantasy. And - without wanting to be too much of an asshole, I'd say that more contemporary English countrysides are... not distinctive enough to be actually all that exploring-worthy.
But yeah, very few companies are actually bold and smart enough to make games based strongly enough on their own cultural legacy. Slavs tend to do that (TW, Stalker, Cryostasis, Pathologic, older ARMA titles, but also shit like Amanita games). Americans tend to use their own famous landmarks too. I know there was this one pretty fantastic Candian game that got little attention, called Sang Froid that was really utterly and thoroughly Candian.
But I do miss more culturally "distinctive" games out there.
