Chalice dungeons are entirely removed from the story, and there are some bosses in the game that you won't see unless you go through one of them, but ultimately optional.

Actually, there's a lot of implications that gaining Insight literally does cause you to mutate and become some kind of monstrous eyeball monster. Umbilical Cords have eyes all over them, for example, and the Brain of Mensis does more damage based on your Insight level because it locks more of its eyes with you. So while the philosophical implications of Insight certainly stand, there's also a good dose of body horror.

Of course the Hunters don't see that themselves because the eyes are on the inside, that or because they're figments of imagination bound to a Nightmare.

I think Micolash and the others purposefully took themselves into the nightmare, along with the human conduits they kidnapped to serve as fodder for The One Reborn, but just failed to pull themselves back out in time to prevent starvation and death. Or, perhaps, they knew they were going to suicide. We'll never know. As for the odd statues and architecture, we see the same thing in Old Yharnam and I think those are just meant to be fucked up decorations, I don't think the people were turned into living statues Pompeii-style.
