Just keep your distance when he transformers, and pick and choose when to attack. Think of playing that part of the fight defensively rather than on the offensive.

tiny music box stuns him and gives you an easy backstab. after you use it 3 times though he'll always transform into a werewolf regardless of how much health he has left.
Once he's a werewolf, the music box will only work on him once, then he's immune.

Also, he's weak to fire.

It's easiest to parry him when he does his roll attack, just shoot as soon as he rolls at you.

Certain tombstones don't break, like the ones to the right. Catch him up on those so he starts swinging wildly, either shoot him mid-swing or parry him and wait for him to berserk. When he berserks read his attacks and repeat the strategy before with a bigger emphasis on keeping distance and stamina.

If you have a weapon with good range like the axe you can attack him while hiding behind the unbreakable tombstones.
But really, just parry him. It's simpler and faster.
