We don't know how they are, it might not be a proper and formal marriage and relationship that Naruto and Hinata or the rest have, and more just a mutual agreement since she got herself knocked up by him (somehow).
There was a joke floating around that he just mailed her a full turkey baster so she'd quit pestering him about having his children.

He certainly didn't give a damn enough to return for twelve years, and likely didn't even have photos of Sarada since his first reaction at seeing her was drawing a sword, thinking she was an enemy.
Plus this is what the autistic fuck looks like once he is forced by circumstance to spend an actual afternoon with his family, and once that is done and he sets out again for possibly years, he still doesn't kiss her.

I really loathe Sakura, but honestly if this shit wasn't so hilarious I'd probably feel sorry for her.
