The first one to do it hardcore was the 80s The Fly for me, and it wasn't all the rest of it as much as it was the birthing scene. I'm over it now obviously but the young me was fucked up.

Return of the Jedi
Vader's face revealed spooked the shit out of me and for a while I would close my eyes during that scene.
Temple of Doom when the bad guy ripped out the guy's heart and then sent him to the fire pit.

Empire Strikes Back or Little Showdown in Big Tokyo
Probably the exorcist
Poltergeist. I saw it when I was VERY young and the scene where the guy pulls his face off in the bathroom mirror haunted me for weeks afterwards. I was scared to go into the bathroom and would literally hallucinate that scene in front of my eyes when I was alone.
wheelchair scene from ninth gate

American Werewolf in London transformation scene. It's tame now, but for a little kid, fuck me. Don't think I slept for a week.
I was fairly young when i watched this scene and i reacted pretty badly.
My mom loved horror movies, still does. Every Halloween instead of trick or treating we'd get soda pop, snacks, and go to the video store and get some movies. The first few years of this were fine and a lovely tradition. Then I turned ten years old and angrily demanded to watch "real" scary movies like her and my father watched when I was asleep. In my mind I wasn't a kid anymore and clearly deserved to watch whatever my parents got to.
Let me sidetrack here for a moment, and ask: have you ever tried to smoke when you were a kid? When I was fourteen I saw my grandfather smoking a cigar and asked to try one. He told me it was a bad habit and not anything a smart man would be doing, much less a child. Much like the horror movie lesson, I became indignant and insisted he give me a cigar. He instead pulled some chewing dip out of his pocket and insisted I try some. I had to hold the dip in my cheek for an hour or else I'd get the switch. It numbed my mouth and made me want to vomit. I never tried smoking and to this day the smell of tobacco makes me absolutely sick. It seems cruel in retrospect but I appreciate the lesson it taught me.
My point is that what my parents had in mind was a similarly mean-spirited way to teach me a lesson. While up to this point we had been watching a healthy blend of stuff like "Gremlins" and "The Nightmare Before Christmas", they opted to turn the dial to 100% and rent a dozen or so movies that were their personal favorites.
That Halloween, as we'd done so many Halloweens before, we got soda pop and snacks and movies and all settled down on the couch. They put in "The Thing" first. Some of the jumpscares spooked me, but I felt like I was pacing myself well and got cocky.
Then we watched A Nightmare on Elm Street. I'm getting close to the character limit here, so I'll just conclude by saying I never asked to see an "adult" horror movie ever again. I had nightmares from that movie until I was 19.

Yeah, Nightmare on Elm Street scared the fuck out of me when I was a kid.

Face Off. Not just the scene where they swap faces but rather the psychological bullshit later on when Cage is in Travoltas body, living his life and whatnot. Messed me up for quite some time.
