I think he just went from one dream into another, that's what the realization is, that things are changing and he'll have to leave.

Think of the Phillip Jeffries scene in Fire Walk With Me, and then The Return, Cole claims he forgot it ever happened "He was never here!" it took him 25 years to remember it, and he did so in a dream.

I think the same thing happened to Cooper, whatever place he was at, is as "real" as "real" is in this show, it wasn't something construed by Judy or himself, people will continue existing there but he will move on.

Sarah sees the white horse before she passes out, she knows what is happening in their house but refuses to accept it. Cooper looks at the white horse after he sees the corpse, he knows on some level that what he's doing is wrong, that he can't fix this but he refuses to acknowledge it.

Cooper also sees the horse in the lodge immediately after Laura whispers into his ear and he looks shocked, he was not ready to hear what she had to say, sees the horse and goes back to starting positions.

The superimposed face appears after Cooper sees Naido - the one character who seemingly has no reason to be there and Coop can't explain how she got there.

Cooper looks at the clock - the clock is stuck in a position that doesn't make sense (minute hand says it's 10 minutes to, but hour hand is past three) - after this he says "we live inside a dream" and the scene dissolves.

I also think the "universe" with Richard and Carrie is another dream. After Coop/Richard walks out of the motel he sees the exterior and car are different than the ones he saw when he entered. But the interior of the motel was the same. That shit only happens in dreams. He seems confused for a second but then just continues on his way, as if he just accepts nothing is real by this point.

As much as the final moments of the show are my favourite moment in cinema/tv all year because of the build up then release of utter existential dread and confusion, I just cannot fully get on board with Lynch saying the whole show was a dream.
