Not every nation needs a big city, that's a relatively modern view. Middle age British kingdoms were barely more than fields and forests and so didn't need massive seats of power, just a formidable army to keep the peasants in line.

The Rohirrim are not nomadic so much as they are rural. Would you call a Cowboy rancher a nomad? No. He has a farm, but he herds his cattle over wide swathes of land.

The Rohirrim are everything Anglo-Saxon, except they have knightly combat, like the Normans. The society is very rural and dispersed, and messengers must be sent around to collect men for fighting -- just as was done in Anglo-Saxon England. The Saxon kingdoms did not have a standing army. The fyrds were made up of men conscripted from the countryside. It just so happens that the Rohirrim all come with their own horses and arms/armor.

This is also something inspired from undeniably English identity -- the demand that all Englishmen be armed and trained in the sword & bow (which eventually inspires the American Second Amendment) because medieval English armies were not only made up of knights, but lowborn serjeants and longbowmen.

The only "Standing Army" of an Anglo-Saxon king would be his Housecarls -- an English version of the German Comitatus. Men with the best arms and armor who protect the king and live at his generosity. These are men like Hama and Gamling -- the well-armored royal guards which live in and around the Meduseld in Edoras.

Look at Wessex in the 700s/800s. It wasn't much different. This is what Anglo-Saxon England was like.

If you take a step back and look at Middle-Earth through the lens of an English Mythology, a lot of it becomes easy to map onto history (this is not to say they're analogies). The Shire is the idealized England of Tolkien's youth. Pastoral, but not rural, with townships and farmers and beer and merry-making. Rohan is Anglo-Saxon England. Gondor is a quasi-Arthurian (the Return of the King, anybody?) country created by the descendants of Atlanteans.

Elves and Dwarves and Orcs are creatures out of the Scandinavian/Germanic mythologies of England.

The entire Kingdom of Gondor stood... and was actually resurgent. Osgiliath, formerly the capital of Gondor, had been retaken by Boromir. Ithilien was once again colonized by Gondorians (like Faramir's Rangers). Rohan was essentially in peace.

Sauron's gains in the Second Era had been great, but not insurmountable. The greatest collapse prior to LOTR is the fall of Arnor, which happened slowly, over time, and during Sauron's period of hiding, almost entirely at the hands of the Witch King. Even so, Glorfindel and the armies of Gondor BTFO the Witch King.

This. Outside of the comfy little Hobbit story the once powerful empire of men has been crumbling for decades. Go for has been without a king for a long time and is struggling to hold it's own against Mordor. It's actually explained a bit in Fellowship between Bilbo's party and when Gandalf returns - that whole series of scenes seems like a few days but it's actually several months.

Middle Earth has been fucked for a long time, which is why the elves are fucking off to their homeland. Everyone sees Mordor's domination over the continent as an inevitability. And even if they stop Sauron, Men will kill themselves off in civil war anyway.
