The final scene is basically the last dream sequence, but its a day dream.

The scene begins with Tony walking into the restaurant, followed by a close up shot of his face, immediately cuts to Tony sitting down at the booth. According to the rhythm of how the scene is shot, it goes

>Shot showing what Tony is looking at
>Another shot of Tony's face reacting to what he's looking at

According to this Rhythm, Tony in the beginning is looking at himself. It's Bucco who praises Tony for his ability to think 20 steps ahead of everybody else, so it's just him planning ahead for what should just be a mundane dinner with his family. The lyric of the song being played when he walks in the diner is "All, all that you dream"
That's why the Journey song he chooses is perfectly timed to what's happening around him.

The ending was heavily foreshadowed in Season 2. Chris goes into a coma and sees his dead father and Mikey Palmice in "purgatory". Mikey asks Chris to tell Tony and Paulie "3 o'clock"

Also, in Funhouse, when Tony wakes up from a bad dream and he's sick from food poisoning, he starts saying "it's all a big nothin, it all goes black"
