I literally only just watched this yesterday, it was kinda schizophrenic for the most part but it was decent when it wasn't trying to be funny. Claire Danes was a total qt back then as well, mad frumpy now though which is a shame.

The use of it is supposed to be silly. The cunt who does it flips someone off and then refuses to admit that he's flipping someone off because he's too chicken to start a fight, like a kid being caught flipping off the school bully. It's a sign of how stupid the feud has become, when random servants are flipping eachother off even when they're so cowardly that they won't admit to doing it. Starting to use the gesture because of that part, rather than being Italian or getting it from some other source, is pretty weird.

I'd say it was a compensator to reduce how the muzzle climbs when the gun is fired, but in the movie there are no gas ports on top of it; which is how handgun compensators normally work

The only thing it seems to actually do is extend the sight radius (distance between the front and rear iron sights) to being closer to the full length of the barrel, which has been extended a few inches.
This can have a marginal effect on precision of aiming with the iron sights since there is a better indication of where the tip of the muzzle is facing, the further way the front sight is.
But he's using a reflex sight anyway, so there's little point in having it
