The book went into a bunch of one off characters that were in the movie. Johnny fontane gets like three whole chapters and character arc dedicated to him, instead of being a sinatra parody. Sonny's mistress gets shipped of west with Fredo and Johnny too and she gets into wacky adventures with Dr. Whiteboi

It blows me away that John Cazale was only like 6 movies and every single one of them is worshipped as some of the best things ever put to film.
