What's scary is that the entire scene in the diner has the camera nearly floating, and tons of "continuity errors" that subconsciously unsettle the audience. And the man talks about something incredibly terrible he saw in a dream, and then it replays the scene in real time. It's scary because it's the opposite of a jump scare, because we know exactly what's coming. Which is sometimes scarier, like an impending death coming in at slow motion.

This scene didn't happen within the movie's narrative. It's a metaphor for the protagonist confronting her inhumane action. Hence the hobo appearing at the climatic suicide.
