I think there's a misunderstanding of what the X stands for on your part as well because it isn't suppose to symbolize his last kill. The first slash he receives from the fiance doesn't heal and constantly hurts because he has been wracked with doubt, guilt and a miserable existence. The second slash from tomoe herself indicates that she forgives him and gives new hope. Hence the wounds heal and no longer hurt resulting in the scars.

I think 15 or 16 years ago I saw this OVA (must've been 15 or so), I was really into my anime and manga phase. This movie almost got me depressed, it certainly moved me deeply. I remember even getting two or three friends over to show it to them.
